Phase 1: Start the Process

  1. Complete and submit the Internship Application Form

  2. The application will be reviewed within two weeks, and a cost will be provided.

Phase 2: Required Documents to Determine Eligibility

  1. Education transcripts

  2. Proof of Identification: Passport, Driver’s License, ID Card

  3. Essay (250-500 words) explaining why student would like to enroll in course/internship.

  4. Background or criminal record check. The criminal record check is free and completed at a local police station. A copy of prior background check within 6 months is acceptable.

  5. A Student evaluation form will be sent out to professors/ instructors listed on application. Once we receive these back you will get a formal letter of acceptance or rejection.

  6. Once accepted into the program, student can initiate the CKD course. Registration and course fee must be paid prior to course initiation.

Phase 3: Submission of Additional Documents

  1. Program competencies and evaluation forms

  2. Vaccination/Immunization Records (for in-person renal internships only)

  3. Travel Medical Insurance (www.internationalstudentinsurance.com)

  4. Health Questionnaire

  5. Food Check List

Phase 4: Final Documentation and Training

  1. Submit certificate of course completion. Course should be finished two weeks prior to start date for internship. Some flexibility will be allowed depending on the situation; however, students will not be allowed to start internship without the certificate.

  2. Final fees must be paid 90 days prior to start of internship. See fee structure and due dates.

  3. Zoom orientations will be scheduled within 6 weeks of the start of the rotation.
